Sample Interview Questions and Answers

Question 1: How would you ensure the accuracy and completeness of patient records during a quality assurance audit?

During a quality assurance audit, I would first review the patient's record to identify any discrepancies or inconsistencies. This includes verifying that all required information is present, such as diagnoses, medications, and laboratory results. Next, I would compare the electronic health record (EHR) with the paper records to ensure that they are identical and up-to-date. If any discrepancies are found, I would research the issue and make corrections or adjustments as needed. Additionally, I would review the audit trail for any changes made to the record and investigate if necessary. Finally, I would update the record accordingly and document the findings in a quality assurance report.

Question 2: What steps would you take to abstract, compile, and enter patient information into statewide and department databases?

To abstract, compile, and enter patient information into statewide and department databases, I would first ensure that I have access to the most current and accurate data from multiple sources, including electronic health records (EHRs), laboratory results, and medications. Next, I would use a standardized abstraction tool or software to extract relevant information from each source. This includes demographic data, medical history, diagnoses, treatments, and outcomes. Once abstracted, I would compile the data into a single dataset for each patient, ensuring that it is accurate and consistent with existing policies and procedures. Finally, I would enter the compiled data into the statewide and department databases using secure and authorized interfaces.

Question 3: How would you assist with responding to medical record requests in compliance with policies, laws, and requirements?

To assist with responding to medical record requests, I would first verify that the request is legitimate and compliant with applicable laws and regulations. This includes ensuring that the request is signed by a competent authority or has been properly authorized for release. Next, I would review the patient's record to identify any sensitive or protected health information (PHI) that may need to be redacted or excluded from disclosure. If necessary, I would also obtain additional documentation or verification of identity and intent before releasing the requested records. Finally, I would assemble and prepare the released records for transmission in a secure manner, such as scanning and encrypting them as required by law.
