Sample Interview Questions and Answers

Question 1: Describe how you would apply the ASAM Criteria and DSM-5 in conducting a substance use disorder assessment for an incarcerated individual, including how you would address complex or conflicting information from collateral sources.

When conducting a substance use disorder assessment, I would first use the ASAM Criteria to evaluate the individual's clinical, medical, and social factors, ensuring a placement decision that aligns with their level of care needs. Concurrently, I would reference the DSM-5 to diagnose any co-occurring mental health disorders. If collateral information from staff or family is conflicting, I would cross-verify details through multiple sources, document discrepancies, and apply critical thinking to determine the most accurate assessment. For example, if an individual's self-reported use contradicts a urine screen result, I would investigate potential reasons (e.g., medication use, false negatives) and consult with medical staff to resolve the conflict before finalizing the assessment.

Question 2: How would you ensure compliance with 42 CFR Part 2 when handling confidential treatment records for incarcerated individuals, particularly when coordinating release services with community partners?

To comply with 42 CFR Part 2, I would strictly limit access to treatment records to only those directly involved in the individual's care, ensuring that all staff handling the records are trained on confidentiality requirements. When coordinating release services, I would use secure methods such as encrypted email or secure portals to share necessary information with community partners, avoiding any disclosure of protected health information (PHI) unless explicitly authorized by the individual. I would also maintain detailed audit logs of all document transfers and ensure that all community partners sign confidentiality agreements before receiving any treatment-related data.

Question 3: Explain your approach to identifying and resolving barriers to completing a substance use disorder assessment, such as an incarcerated individual's reluctance to participate or incomplete collateral information.

If an individual is reluctant to participate, I would first build rapport by explaining the assessment's purpose, emphasizing its role in tailoring treatment to their needs and improving outcomes. I would also involve peer support staff or mental health counselors to encourage participation. For incomplete collateral information, I would systematically follow up with relevant stakeholders (e.g., classification officers, medical staff) to gather missing data. If information remains unavailable, I would document the gaps, use available data to make an informed assessment, and flag the case for follow-up once additional information becomes accessible. This approach ensures that assessments are completed promptly while maintaining accuracy.
