Sample Interview Questions and Answers

Question 1: Describe how you would assess and manage a patient presenting with symptoms suggestive of a chronic disease like diabetes within the corrections facility environment. What tools or assessments would you use?

Assessing and managing a patient with suspected diabetes involves several steps. First, I'd conduct a comprehensive health history interview to understand any existing conditions, medications, dietary habits, and family history of diabetes. Next, I would perform a physical examination focusing on signs associated with hyperglycemia or hypoglycemia, such as dry skin, frequent urination, excessive thirst, blurred vision, and fatigue. Key assessments include measuring blood glucose levels using a glucometer, conducting a Hemoglobin A1C test to evaluate long-term glucose control, and checking for complications like neuropathy by assessing foot sensation and reflexes. I would also review vital signs and conduct a urine analysis for ketones if diabetes is suspected. Management includes educating the patient on dietary modifications, exercise, medication adherence (if prescribed), and self-monitoring of blood glucose levels. In a corrections setting, where resources may be limited, I'd coordinate with healthcare providers to ensure necessary medications are stocked, arrange regular follow-up appointments, and utilize electronic health records for tracking and scheduling patient visits. Additionally, ensuring that the patient is aware of signs of hypo- or hyperglycemia is crucial for prompt intervention.

Question 2: How would you handle a situation where an incarcerated individual expresses suicidal ideation? Outline your immediate response and follow-up actions.

When an incarcerated individual expresses suicidal ideation, it's critical to act swiftly and compassionately. My immediate response would involve ensuring the individual's safety by removing any potential means of self-harm from their environment. I'd conduct a thorough risk assessment using standardized tools such as the Columbia-Suicide Severity Rating Scale (C-SSRS) to evaluate the severity and immediacy of the suicidal thoughts, including plans or previous attempts. Establishing rapport is essential; thus, I would engage with the individual in a calm and supportive manner, actively listening without judgment to understand their feelings and concerns. After ensuring immediate safety, I'd collaborate with mental health professionals for further evaluation and intervention, which might include counseling or psychiatric consultation. Documentation of all assessments, observations, and actions taken is vital for ongoing care continuity. Follow-up involves creating a safety plan in collaboration with the individual, setting up regular check-ins by healthcare staff, and ensuring that they have access to crisis support resources both within the facility and after their release if applicable.

Question 3: Given your role as an on-call nurse at Stafford Creek Corrections Center, how would you prioritize tasks during a shift when multiple health emergencies arise simultaneously?

Prioritizing tasks during a shift with concurrent health emergencies requires triage to ensure the most critical patients receive immediate attention. First, I'd perform a rapid assessment of all presenting emergencies using the ABCDE approach (Airway, Breathing, Circulation, Disability, Exposure) to identify life-threatening conditions such as respiratory distress or cardiac events. Patients in need of urgent intervention for maintaining airway and circulation would be prioritized first. Following this initial triage, I'd consider factors like the severity of symptoms, potential for rapid deterioration, and available resources when deciding on next steps. Communication is key; thus, I'd coordinate with colleagues to delegate tasks based on their expertise, ensuring that each patient receives appropriate care promptly. For example, a nurse colleague might manage a non-critical but time-sensitive condition while I handle an acute emergency. Documentation throughout this process helps track actions taken and inform subsequent care decisions. Continual reassessment of all patients is crucial as situations can evolve quickly. Additionally, maintaining clear communication with the healthcare team and inmates' families, where appropriate, ensures transparency and support for everyone involved.
