Question 1: Describe a specific instance where you had to implement infection control techniques while assisting a resident. How did you ensure compliance with protocols and maintain a safe environment?

In my previous role at a long-term care facility, I consistently followed infection control protocols by practicing proper hand hygiene before and after resident care, using personal protective equipment (PPE) such as gloves and gowns during procedures, and properly disposing of contaminated waste in designated containers. When a resident showed signs of a potential infection, I immediately isolated them in a private room, reported the situation to the charge nurse, and ensured all staff members were briefed on the incident to prevent cross-contamination. This approach helped maintain a safe environment and minimized the risk of infection spread among residents.

Question 2: How do you ensure accurate documentation of resident data, such as vital signs and intake/output records, and why is this important for patient care?

Accurate documentation is critical for ensuring continuity of care and identifying changes in a resident's condition. In my experience, I used electronic health records (EHR) systems to record vital signs, intake/output, and other observations promptly and precisely. I double-checked measurements with a colleague when discrepancies arose and ensured that all entries were clear, legible, and completed in real time. For example, when a resident's urine output decreased significantly, my timely documentation allowed the nursing team to intervene early, preventing potential complications such as dehydration or urinary retention.

Question 3: Explain how you would handle a situation where a resident becomes agitated or combative during a routine care task, such as bathing or medication administration.

First, I would remain calm and ensure the resident's safety by creating a non-threatening environment. I would use de-escalation techniques, such as speaking in a soothing tone, offering reassurance, and allowing the resident time to process their emotions. If the resident's behavior escalated, I would immediately seek assistance from a licensed nurse or security personnel while ensuring I did not attempt to physically restrain them. In a previous situation, I successfully de-escalated a resident's agitation by involving their family member in the care process, which helped the resident feel more secure and cooperative.

*This position has been posted within the assigned pay range through Step L. Step M has not been published as this is considered the longevity step, in alignment with state HR compensation rules.

Updated as of July 1, 2025: This position is eligible for "Supplement Basic Shift Premium" for  24/7 Facilities. Employees who are assigned to a facility that provides direct care to residents, patients, and/or clients and whose duties are required to be performed on location will receive an additional one dollar ($1.00) per hour of supplemental basic shift premium for each hour of basic shift premium. This shift premium does not apply to overtime hours. The supplemental basic shift premium shall expire on June 29, 2027.

As a Nursing Assistant Certified, you will assist in the delivery of nursing and nursing-related care to residents of the Washington Veterans Home in accordance with current standards of practice.

Duties

Some of what you'll do:

  • Assist residents with bathing, dressing, grooming, hygiene care, toileting, eating, and assisting with the monitoring/feeding of meals and nourishments.
  • Assist with range of motion exercises, practice infection control techniques, and observe for and report changes in resident condition to licensed personnel.
  • Accurately obtain and record vital signs and other resident data.
  • Document intake and output records.
  • Perform basic nursing procedures such as applying anti-embolism stocking, applying creams or lotion to intact skin, etc.
  • Dispose of contaminated waste as appropriate.
  • Maintain a safe, sanitary, and harmonious environment.
  • Clean bedpans, washbasins, urinals, commodes, beds, night stands, and over bed tables.

Qualifications

Required:
  • A combination of education and experience may be considered on a year-for-year basis. Experience must be relevant to the position.
  • Valid Washington state license to practice as a nursing assistant certified.
  • Must be proficient in the English language and be able to read, write and speak English.
  • Active status on Omnibus Budget Reconciliation Act (OBRA) Registry.
  • Ability to:
    • Perform data entry.
    • Maintain confidentiality at all times.
    • Provide a positive living environment for residents relating to meeting psychosocial needs; follow written and verbal directions; document and report observed changes in patient condition and behavior.
    • Demonstrate proficiency in the English language and be able to read, write and speak English. 
Preferred/Desired:
  • Long-term care experience.
Special Requirements:
  • Must be able to pass a DSHS BCCU Background Check prior to beginning services and every two (2) years in accordance with WDVA Policy 615.000 Background Checks. Information from the background check will not necessarily preclude employment but will be considered in determining the applicant's suitability and competence to perform in the position.
  • Must comply with annual Tuberculin Screening in accordance with DVA policy 670.000 Tuberculosis Testing.
  • Requires medical evaluation, fit testing, training, and use of respiratory protection as required by the Washington Industrial Safety and Act standards. Must be able to wear a respirator, medical gloves, gowns, eye protection, and other personal protection as required by a job or task.
  • Must be able to lift up to 60 lbs. and be capable of reaching a minimum of 36" using proper technique to stock shelves and access supplies. Employee must also be able to push or pull with sufficient strength and walk, sit, and bend in an unrestricted manner during the entire workday.
  • Must remain current on the OBRA Registry.
  • Must maintain a valid NAC license with the Washington State Dept. of Health.
  • Must remain current in CPR.
  • Must maintain regular and reliable attendance and be willing to work as needed.
  • Must successfully complete, within mandated timeframes, employee orientation and all other mandatory annual, in-service, and other required training.
  • Must become familiar and comply with all DVA policies and procedures and Collective Bargaining Agreements as applicable.
  • Must sign the confidentiality statement for all employees.
  • Positions requiring travel must have a valid unrestricted driver's license and must be insured if driving a privately owned vehicle on state business.
  • Candidates who are offered a job with WDVA must possess work authorization which does not require sponsorship by the employer for a visa now or in the future.
